Symptoms of SSPE

The initial symptoms of SSPE are subtle and include the following
Mild cognitive decline (such as memory loss)
Changes in behavior (such as irritability)
Disturbances in motor function, including uncontrollable jerking movements of the head, trunk or limbs called myoclonic jerks
Seizures may occur
Some people may go blind

Clinical stages of SSPE

Stage I-

Personality or behavioral
changes

Irritability, dementia, lethargy, social
withdrawal, or speech regression

Stage II-

Progressive decline in
motor function

Various types of movement disorders, like dyskinesia,
dystonia, and myoclonus

Stage III-

Extrapyramidal
symptoms

Posturing and Spasticity

Stage IV-

Loss of function of
cerebral cortex

Patients develop akinetic mutism, autonomic failure,
or enter into vegetative state

Diagnostic methods

The clinicians tend to use a set of major and minor criteria for diagnosis of SSPE.

Major criteria

  • High measles-specific antibody titers in the cerebrospinal fluid (CSF)
  • Typical or atypical clinical history
  • Typical: acute (rapidly) progressive; subacute progressive, chronic progressive, chronic relapsing-remitting.
  • Atypical: seizures, prolonged stage I,
    unusual age (infancy/adult)
    at presentation.

Minor criteria

  • Typical EEG (periodic complexes)
  • Increased cerebrospinal fluid IgG
  • Brain biopsy findings consistent with SSPE
  • Special molecular diagnostic test to
    identify measles virus mutated genome.
